ABSTRACT

Objective To discuss the safety and the application of the self-designed multifunctional inflatable pelvis and hip-joint fixator (MIPHF) in damage control in pelvic fracture patients. Methods The MIPHF was subjected to pressure test and quality inspection. From September 2016 to June 2017,61 pelvic-fracture patients were treated with our self-designed MIPHF as pre-hospital first-aid care according to the concept of damage control orthopedics (DCO) (MIPHF group). The control group consisted of 69 pelvic-fracture patients who had not received pre-hospital first-aid care with the self-designed MIPHF from December 2015 to August 2016.There were no statistically significant differences between the two groups in gender, age, types of pelvic fracture, and preoperative injure severity score (ISS).The study compared the two groups for the case fatality rate, volume of blood transfused during surgeries, early complication rates, fracture reduction (Matta standards),and long-term efficacy (Cole scores). Results The pressure test showed that the MIPHF had a good fixation effect on the pelvis. And the quality inspection showed that the material used for the MIPHF was in line with national standards and the safety was guaranteed. The MIPHF group had 1 death (1.6%) and the control group had 8 deaths (11.6%),which was a significant difference (x2=4.979,P=0.026).All survival patients in both groups were followed up.The MIPHF group (61 cases) received 3.0 to 18.0 months follow-up,with an average of 9.0 months. And the control group (69 cases) had 18.0 to 30.0 months follow-up,with an average of 21.9 months. In the MIPHF group,23 cases were treated conservatively, and 37 cases were treated with surgery.Among them,3 cases were fixed with external fixator,20 cases with anterior open reduction and internal fixation,9 cases with posterior open reduction and internal fixation, and 5 cases with combined anterior and posterior fixation. The timing of surgery was 1 to 20 days after injury, with an average of 4.1 days. The volume of blood transfused in the MIPHF group during surgery was 200 to 1500 ml,with an average of 628.6 ml.In the control group,27 patients were treated conservatively, and 42 patients were treated with surgery.Among them,2 cases were fixed with external fixator,24 cases with anterior open reduction and internal fixation,10 cases with posterior open reduction and internal fixation, and 6 cases with combined anterior and posterior fixation. The timing of surgery was 1 to 15 days after injury, with an average of 3.l days. The volume of blood transfused in the control group during surgery was 200 to 4000 ml,with an average of 1 707.1 ml.There was a significant difference between the two groups in intraoperative blood transfusion(Z=-2.330,P=0.020).The MIPHF group had 10 (16.4%) cases of early serious complications and the control group had 22 (31.9%) cases, which had a significant difference (x2=4.187,P=0.041).According to the criteria proposed by Matta et al.,the good rate of results for treating fractures was 82.0% in the MIPHF group and 60.9% in the control group, which got a significant difference (x2=6.967,P=0.008).The MIPHF group and the control group also differed significantly in their mean long-term Cole scores (27.2±4.0 versus 25.1 ±5.6,t=2.457,P=0.015).Conclusion MIPHF,which reflects the DCO concept, may be recommended as pre-hospital first-aid care for patients with pelvic fracture because it can lessen bleeding and prevent secondary pelvic injury, thus reduce case fatality rate and the incidence of complications. It can also improve the success rate of treating pelvic fracture, which will positively affect long-term outcomes.

ABSTRACT

BACKGROUND:Poly(3-hydroxybutyrate) is approved as its excel ent biocompatibility, biodegradability and piezoelectric properties, but there are also some deficiencies, such as high breakability and poor hydrophilicity. METHODS:Poly(3-hydroxybutyrate) was mixed with different mass percentages of nanohydroxyapatite (0, 10%, 20%and 30%) to prepare new composite fibrous scaffolds through electrospinning process. The microstructure, group composition, crystal ine phase distribution, thermal properties and surface wettability of the scaffolds were detected. RESULTS AND CONCLUSION:Under the scanning electron microscope, with the increase of nano-hydroxyapatite content, more and more nano-hydroxyapatite particles were distributed evenly on the composite fiber surface;the fiber surface was basical y covered with nano-hydroxyapatite particles at the content of 30%, and the roughness of the fiber surface also increased. Results from differential scanning calorimetry and X-ray diffraction showed that the nano-hydroxyapatite reduced the crystal inity of poly(3-hydroxybutyrate) and the crystal tacticity, and this phenomenon became more evident with the increase of nano-hydroxyapatite content. Additional y, the higher the content of nano-hydroxyapatite content, the lower the contact angle and the higher the hydrophily. These findings indicate that the nano-hydroxyapatite/poly(3-hydroxybutyrate) ultrafine-fibrous scaffold using electrospinning technology can effectively improve the surface wettability and crystal inity of the material as wel as the material hydrophily and brittleness, and the higher the content of nano-hydroxyapatite, the more obvious the effect.

ABSTRACT

In this study, we obtained the TiO2 films with different thickness by micro-are oxidation (MAO) on commercially available pure titanium. By altering the duration time, we got the films 5 microm, 10 microm, and 20 microm in thickness, respectively. XRD and SEM were employed to characterize the phase, composition and microstructure of the films. The MAO film, which was about 5 microm thick, was porous and even with the pore size about 1 microm. No other crystalline phase except anatase was detected by XRD. The film, which was 10 microm thick, was similar to the former. The pores were well separated and homogeneously distributed over the surface. The XRD pattern indicated that the film was composed of anatase and a minor amount of rutile, whereas the pore size of 20 microm film was about 4-6 microm and was bigger than the others. Furthermore, the small cracks were easy to observe. XRD pattern showed that the Ti peak was significantly reduced and new Ca, P, O containing compound was formed in addition to rutile and anatase with the film thickness increasing. There were significant differences in regard to surface roughness of the three groups. These findings suggest that the film thickness has an intense effect on the quality of the MAO coating. By changing the film thickness, we can extensively change the composition, structure and chemical properties of the surface layer on titanium.

ABSTRACT

Hydrothermally synthesized nano-hydroxyapatite (n-HA) varying in weight from 10% to 30% was used as filler to make nanocomposites with novel aliphatic polyesteramide (PEA) in our laboratory. The structure and properties of PEA and its n-HA composites were investigated through transmission electron microscopy, infrared spectrometry, X-ray diffractioin, scanning electron microscopy and energy spectrometry. The shape and size of the n-HA crystals are similar to those of the apatite crystals in natural bone. Molecule interactions are present between the n-HA and PEA in the composite, which allows the uniform dispersion of n-HA in PEA matrix. This contributes enhanced mechanical property and bioactivity to the composite. The cytocompatibility of the composites has been investigated by culturing osteoblasts on the membranes. Good cell attachment and proliferation manner were observed on the membranes after 1 week. These results suggest that the PEA/n-HA composites prepared in this study may serve as potential candidate scaffold for tissue engineering.

ABSTRACT

In order to study the cytocompatibility of nanophase hydroxyapatite ceramic in vitro, we prepared hydroxyapatite by use of the wet chemistry techniques. The grain size of hydroxyapatite of interest to the present study was determined by scanning electron microscopy and atomic force microscopy with image analysis software. Primary culture of osteoblast from rat calvaria was established. Protein content, synthesis of alkaline phosphatase and deposition of calcium-containing mineral by osteoblasts cultured on nanophase hydroxyapatite ceramics and on conventional hydroxyapatite ceramics for 7, 14, 21 and 28 days were examined. The results showed that the average surface grain size of the nanophase and that of the conventional HA compact formulations was 55 (nanophase) and 780 (conventional) nm, respectively. More importantly, compared to the synthesis of alkaline phosphatase and deposition of calcium-containing mineral by osteoblasts cultured on nanophase was significantly greater than that on conventional ceramics after 21 and 28 days. The cytocompatibility was significantly greater on nanophase HA than on conventional formulations of the same ceramic.

ABSTRACT

OBJECTIVEThe purpose of this study was to determine the effects of amino group implantation on the surface structures of titanium.

METHODSThe amino groups were implanted into the surfaces of titanium using 100 keV amino group ions at ambient temperature (25 degrees C) with the following three different doses: 5 x 10(17), 1 x 10(17) and 5 x 10(16) ions per cm2. The current density of the ion-beam was (10 microA/cm2. The modified surfaces were characterized using X-ray photoelectron spectroscopy (XPS) to determine the chemical components of the modified surfaces. A scanning electron microscope (SEM) was used to study the physical structure of the surfaces.

RESULTSThe wide XPS profiles, the binding energy data and the SEM photos were obtained. The XPS showed that the modified surface contained Ti, O, C and N. The possible chemical states were speculated using Gauss matching analysis. There was no significant difference between the surfaces with and without implanted amino groups.

CONCLUSIONThe XPS data reveals that the structure of the modified surface layer of aminogroup-implanted titanium is TiN, comprising a complex structure of titanium and amino compound near the surface. The SEM Photos show that this technique has little effect on the surface structures of materials.

ABSTRACT

To elucidate the adsorption mechanism of CS-A to the surface of titanium, 5 ml solutions of the CS-A were reacted with 2 g of native and 2 g of calcium-treated titanium powder for 48 h at 37 degrees C. Residual CS-A was detected by the carbazole elaborate method. The results showed that no CS-A attached to native titanium. Comparatively, titanium treated with calcium produced a significant adsorption of CS-A. At concentration of 60 micrograms/ml, the adsorption of CA-A to calcium-treated titanium powder attained the maximum, 83 micrograms/g. Only EDTA can liberate the bound CS-A from titanium surface. These findings suggest that calcium ion is necessary for the adsorption of CS-A to titanium.

ABSTRACT

TF no-mix orthodontic adhesive was compounded in different weight proportion of nanograde hydroxyapatite as 10%, 20% and 30%, for use in this comparative study on composites. 80 orthodontic brackets were cemented and divided into 8 experimental groups, and TF adhesive was set as the control group. The tensile strength and shear strength were tested respectively. The results showed that the group of 10% nanograde hydroxyapatite obtained the greatest bonding strength (9.668 +/- 1.0649 MPa in tensile strength and 11.991 +/- 1.4135 MPa in shear strength), and there was significant difference between the 10% group and the other groups (P < 0.05, one-way ANAOV). It is concluded that the bonding strength can be enhanced when TF adhesive is compounded in suitable proportion of nanograde hydroxyapatitie.

ABSTRACT

Objective: To study the cytocompatibility of nanophase alumina ceramics with osteoblasts. Methods: Alumina ceramics were prepared via wet chemistry techniques. The grain size of alumina of interest in the present study was determined by scanning electron microscopy and atomic force microscopy with image analysis software. Primary osteoblast culture was established from rat calvaria. Protein content, synthesis of alkaline phosphatase(ALP) and deposition of calcium-containing mineral by osteoblasts cultured on nanophase alumina ceramics and conventional alumina ceramics for 7, 14, 21 and 28 d were respectively examined. Results: The average surface grain size of the nanophase and conventional alumina compact formulations was 60 nm and 1.80 ?m respectively.Synthesis of ALP and deposition of calcium-containing mineral were significantly greater by osteoblasts cultured on nanophase than those on conventional ceramics after 21- and 28- day culture. Conclusions: Nanophase alumina may stimulate ALP synthesis and calcium deposition of osteoblasts.

ABSTRACT

Objective To evaluate the anatomical and functional results of retinotomy in treatment of complicated retinal detachment. Methods Twenty three eyes were treated with retinectomy during vitrectomy for complicated retinal detachment with proliferative vitreoretinopathy when complete reattachment of retina was not obtained despite careful mambrane peeling. After released the peripheral vitreoretinal contraction or pulled subretinal membranes, perfluorocarbon liquid injection, laser retinopexy and silicone oil tamponade were performed. Retinotomy size ranged from 30 degree to 360 degree (average 132 degree). Results Retinal attachment were obtained in all of the operated eyes at the end of the operation. Silicone oil was removed from 15 eyes (65.0%) within 3~11 months postoperatively. After a minimum follow up of 6 months, the retinae were completely attached in 17 eyes ( silicone oil was not removed in 4 eyes ) and visual acuity of 0.02 or better was obtained in 11 eye (48.0%). Recurrent retinal detachment occurred in 2 eyes in which the silicone oil had been removed. The major complications were recurrence of the proliferation in 6 eyes (26.0%) and hypotony in 3 eyes (13.0%). Conclusion Retinotomy is beneficial to reattach the retina for eyes with advanced proliferative vitreoretinopathy in seemingly inoperable cases.

ABSTRACT

This paper reports the results of cytotoxicity test of pearls of Pinctada martensii in vitro. The results of relative growth rate (RGR) and observation of cell morphology by putting up extracted solution of pearl powder in culture flask of L929 cell show that pearls of Pinctada martinsii are cytocompatible. The RGR of pearls of Pinctada martensti is lower than that of hy-droxyapatite.
